voting for the first time;
You moved here from another state;
You changed your name;
You moved since you last voted;
You didn't vote in the last General Election;
You want to become a member of the
American Independent Party ?
You've got two options :

help you, as necessary, to fill it out; and deliver it to the Registrar's office for you )
Do it yourself. Go to the Registrar's office
